1. SPONSOR. This Sweepstakes is sponsored by ‘WCIU, The U,’ whose corporate address is 26 N. Halsted, Chicago, Illinois 60661 (the "Sponsor").
2. ENTRY. A purchase is not required to enter or win the Sweepstakes, and purchase does not increase the odds of winning. The Sweepstakes begins Monday, December 10, 2012 3:00 PM CST and ends Saturday, December 15, 2012 11:00 AM CST unless extended by ‘WCIU, The U.’ Entrants have the chance to win one (1) of five (5) $500 gift cards to an undisclosed grocery store (the “Prize”) via the ‘Judge Mathis buys your Groceries’ Sweepstakes. To participate, viewers must watch ‘Judge Mathis’ weekdays at 3:00 PM CST during the Sweepstakes period on ‘WCIU, The U’ for their chance to win. Each day, one (1) Code Word of the Day will be shown during the 3:00 PM CST broadcast of ‘Judge Mathis.’ To enter the Sweepstakes, viewers must submit the correct Code Word by 2:59 PM the following day, except on Saturday, in which the entry deadline is 11:00 AM CST. Only one (1) entry per person, per email address, per day will be accepted. Five (5) winners will be selected using a computer software winner selection tool from all eligible entries.
3. ELIGIBILITY. ‘Judge Mathis buys your Groceries’ Sweepstakes is open to all legal residents of Illinois and Indiana who live in the Chicagoland television viewing area (as defined by A.C. Nielsen) and are 21 years of age or older. Employees of ‘WCIU, The U,’ Weigel Broadcasting Co., Warner Brothers, WVAZ, and their advertising agencies, consultants, franchisees, suppliers, subsidiaries and family members of any of the above are not eligible to enter or win.
4. PRIZES. ‘WCIU, The U’ and Warner Brothers guarantee that five (5) winners will each receive one (1) $500 gift card to an undisclosed grocery store and a meet and greet with Judge Mathis on Monday, December 17 2012 at an undisclosed grocery story in the Chicagoland area, valued at $500. Odds of winning will be determined by the total number of eligible entries. Winners from Monday, Tuesday, Wednesday and Thursday will be selected at 3pm CST the following day except for the Friday winning which will be selected at 11am CST on Saturday, December 15, 2012. Any selected winner who cannot be contacted within 24 hours of the initial phone call to the number listed on their entry to attempt to confirm their selection will be disqualified and an alternate entrant will be selected by random drawing from the remaining eligible entries and notified. Prize winners must possess a valid picture ID to claim their Prize. The exact location of the grocery store will be made available to winners on Monday, December 17, 2012 at 11 am CST by a station representative. Each winner is invited to attend a meet and greet with Judge Mathis beginning at 1pm with four (4) guests. Prizes will be made available at this event and to use after the meet and greet concludes. Winners who are unable to be present will have their Prize delivered to the address on their entry via UPS Ground.
5. PRIZE RESTRICTIONS. These Prizes are non-transferable. Prizes are subject to the terms and conditions of the Prize provider. There will be no substitute or cash redemption awarded to any Sweepstakes winner. All income and other taxes are the responsibility of the winners. By entering, the winners are consenting to the use of their name, likeness, and/or biographical information for advertising and promotional purposes without additional consideration.
6. DISCLAIMER OF LIABILITY. Winners are responsible for income and other taxes on the Prizes. Winners will be required to sign and return an Affidavit of Eligibility, Release of Liability, and Letter of Indemnification within 24 hours of notification of being a winner. Prize winners must complete a W-9 form. Failure to comply with the rules and conditions of the Sweepstakes may result in a forfeiture of Prize.
7. MISCELLANEOUS. The decisions made by ‘WCIU, The U,’ concerning any and all matters with respect to this promotion shall be final. In the event of a computer, telephone failure, or any other circumstances beyond the control of ‘WCIU, The U,’ where it becomes impracticable to award one or more prizes, ‘WCIU, The U,’ may, in its sole and absolute discretion, determine not to award any such Prize. This Sweepstakes is void where prohibited, penalized or otherwise restricted by law. A copy of these official Sweepstakes rules are available at the ‘WCIU, The U’ offices located at 26 N. If you wish to receive the names of Sweepstakes Prize winners, you must send a self-addressed, stamped envelope to ‘Judge Mathis buys your Groceries’ 26 N. Halsted, Chicago, IL 60661. ‘WCIU, The U’ must receive any request for the winners’ names no later than April 5, 2013.
