1. SPONSOR. This Sweepstakes is sponsored by ‘Me-TV Network’ whose address is 26 N. Halsted, Chicago, Illinois 60661.
2. ENTRY. A purchase is not required to enter or win the Sweepstakes, and a purchase does not increase the odds of winning. The Contest begins Monday, December 3, 2012 12:00 AM and ends Friday, December 21, 2012 11:00 AM CST unless extended by ‘Me-TV Network.’ Persons wishing to enter the Sweepstakes may do so at MeTVNetwork.com and complete an online form to register for a chance to win one (1) copy of The Dick Van Dyke Show: The Complete Series on Blu-ray or one (1) Grand Prize, The Dick Van Dyke Show: The Complete Series on Blu-ray that is autographed by Dick Van Dyke and Carl Reiner. Only one (1) entry per person, per email address, per day (12 AM – 11:59 PM CST) will be accepted. If multiple entries are received from one entrant in any given day during the Sweepstakes period, that day’s entries for that particular entrant will not be accepted. Twelve (12) winners will be selected at random from all online entries using a computer software winner selection tool.
3. ELIGIBILITY. The Sweepstakes is only open to all legal residents of the contiguous 48 states, Hawaii and the District of Columbia who are 18 years of age or older. Employees of ‘Me-TV Network,’ MGM Television, Weigel Broadcasting Co., SPPR, Inc., Image Entertainment, sponsors and each of their advertising agencies, consultants, franchisees, suppliers, subsidiaries and family members are not eligible to enter or win. Entrants are not able to win more than one (1) time during this Sweepstakes. This Sweepstakes is void where prohibited by law.
4. PRIZE. ‘Me-TV Network,’ and Image Entertainment guarantee that 11 winners will receive one (1) copy of The Dick Van Dyke Show: The Complete Series on Blu-ray, retail value of $349.98; one (1) Grand Prize winner will receive The Dick Van Dyke Show: The Complete Series on Blu-ray that is autographed by Dick Van Dyke and Carl Reiner, retail value of $349.98. The last winner selected will receive the Grand Prize. There are twelve (12) total winners in this Sweepstakes. Odds of winning will be determined by the total number of Sweepstakes entries. Winners will be selected and notified by the following timeline:
- One (1) winner will be selected on Friday, December 7, 11am CST
- One (1) winner will be selected on Monday, December 10, 11am CST
- One (1) winner will be selected on Tuesday, December 11, 11am CST
- One (1) winner will be selected on Wednesday, December 12, 11am CST
- One (1) winner will be selected on Thursday, December 13, 11am CST
- One (1) winner will be selected on Friday, December 14, 11am CST
- One (1) winner will be selected on Monday, December 17, 11am CST
- One (1) winner will be selected on Tuesday, December 18, 11am CST
- One (1) winner will be selected on Wednesday, December 19, 11am CST
- One (1) winner will be selected on Thursday, December 20, 11am CST
- Two (2) winners will be selected on Friday, December 21, 11am CST
Winners will be selected by way of a computer software winner selection tool from all eligible entries received during the Sweepstakes period. Any selected winner who cannot be contacted by 4 PM CST to confirm their selection after an initial phone call to the number listed on their entry will be disqualified and an alternate entrant will be selected by way of a computer software winner selection tool from the remaining eligible entries and notified. The Prize winners must possess a valid picture ID to claim their Prize.
5. PRIZE RESTRICTIONS. The Prizes are non-transferable and will be delivered no later than thirty (30) days from the date that such Prize is awarded at the expense of ‘Me-TV Network’ to all winners via UPS Ground with signature required. There will be no substitutes or cash redemptions awarded to the Sweepstakes winners. By entering, the winners shall consent to the use of their name, likeness, and/or biographical information for on-air announcements, advertising and promotional purposes related to the Sweepstakes, without additional consideration.
6. DISCLAIMER OF LIABILITY. The winners are responsible for income and other taxes on their Prize. The winners will be required to sign and return an Affidavit of Eligibility, Release of Liability and Letter of Indemnification upon receipt of their Prize. Failure to comply with the rules and conditions of the Sweepstakes may result in a forfeiture of their Prize. Winners have five (5) business days to return all requested identification and release forms. After this time period, the Prize may be revoked and a new winner may be drawn from all eligible entries.
7. MISCELLANEOUS. The decisions made by ‘Me-TV Network’ concerning any and all matters with respect to this promotion shall be final. In the event of a computer or telephone failure, or any other circumstances beyond the control of ‘Me-TV Network’ where it becomes impracticable for ‘Me-TV Network’ to award the Prize, ‘Me-TV Network’ may, in its sole and absolute discretion, determine not to award any such Prize or any Prize at all. ‘Me-TV Network’ is not responsible for replacing the Prize as a result of events beyond the control of the ‘Me-TV Network.’ This Sweepstakes is void where prohibited, penalized or otherwise restricted by law. Any attempt by any participant to bypass these rules by the use of alias email addresses, illegitimate identities, registrations or logins, or any other scheme will void that participant's entry/entries and may be disqualified from the Sweepstakes. If you wish to receive the name of the Sweepstakes Prize winners, you must send a self-addressed, stamped envelope to ‘Me-TV Network,’ ‘Me-TV Dick Van Dyke DVD giveaway – Sweepstakes Winners,’ 26 N. Halsted, Chicago, IL 60661. ‘WCIU, The U’ must receive any such request for the winners’ names no later than April 15, 2013.
